Gait Training


1. Parallel Bars
  1. Instruction and training in:
    1. Set-to-stand and reverse with or without assistive devices.
    2. Static and dynamic standing balance.
    3. Use of appropriate gait pattern with or without assistive device while progressing forward and turning.
    4. Due to limited space it may not be possible to use an assistive device in standard parallel bars.
    5. Stepping forward, backward, sideward, and turning.

2. Overground Indoors

  1. Instruction and training in:
    1. Appropriate gait pattern and use of the assistive device.
    2. Stepping forward, backward, and sideward with or without resistance.
    3. Cross stepping and braiding
    4. Walking over and around objects (i.e., obstacle course)
    5. Crossing thresholds and entry/exiting through doorways including opening and closing door
    6. Stairs
    7. Dual-task walking
    8. Running


3. Body Weight Support/Treadmill

  1. Instruction and training in:
    1. Stepping on the treadmill using body weight support (BWS) with maximally tolerated lower extremity load-bearing progressing to no body weight support (BWS)
    2. Reciprocal stepping pattern with manual assistance at lower extremities are trunk with normal or near-normal lower extremity and trunk kinematics progressing to no manual assistance
    3. Production of rhythmic stepping pattern with arm swing and minimal to weight-bearing through the upper extremities
    4. Progress stepping speed to normal values based on age
    5. Walk forward, sideward, and backward
    6. Strategies to minimize abnormal/compensatory movement patterns
    7. Strategies to improve aerobic capacity

4. Overground Community

  1. Instruction and training in:
    1. Curb climbing, negotiating ramps, stairs, and sloped surfaces
    2. Walking over even and uneven terrain
    3. Walking within imposed timing requirement (e.g., crossing at a red light, walking onto an elevator, walking through a revolving door)
    4. Walking for long distances
    5. Walking at varying speeds, walk using a rhythmic timing device (e.g., metronome)
    6. Dual-task walking
    7. Waking in open environment with distracters
    8. Entering and exiting transportation vehicles
    9. Running

5. Body Weight Support/Overground

  1. Instruction and training in:
    1. Walking overground with maximally tolerated lower extremity load-bearing progressing to no BWS
    2. Use of assistive device for walking on level surfaces
    3. Reciprocal staffing pattern with manual assistance at this with normal or near-normal lower extremity and trunk kinematics progressing to no manual assistance
    4. Production of rhythmical, coordinated stepping pattern with arm swing and minimal weight bearing through the upper extremities
    5. Strategies to minimize abnormal/compensatory movement patterns
    6. Strategies to maintain balance when perturbed
